Cold water is one of the most underestimated dangers at sea. Even in summer, the water around much of Europe stays cold enough to disable a strong swimmer within minutes. People often assume that drowning in cold water is simply a matter of not being able to swim, but the reality is that the body’s reaction to sudden cold is what most often proves fatal.

Understanding how cold water affects you, and knowing what to do if you or someone else ends up in it, dramatically improves your chances of survival. This guide explains the stages of cold-water immersion and how to respond both in the water and after a rescue.

Why Cold Water Is So Dangerous

Water draws heat from the body far faster than air of the same temperature. Water that feels merely brisk can incapacitate you long before you feel truly frozen. The danger is not one thing but a sequence of effects that unfold over time.

Crucially, you do not need to be in freezing water for it to be deadly. Water in the mid-teens Celsius, common around European coasts even in the warmer months, is cold enough to trigger the dangerous responses described below. Never judge the risk by how pleasant the water looks from the deck.

The Stages of Cold-Water Immersion

Cold-water immersion affects the body in recognisable stages, and knowing them helps you respond correctly.

  • Cold water shock happens in the first minute or two. Sudden immersion causes an involuntary gasp and rapid, uncontrollable breathing, along with a spike in heart rate and blood pressure. Gasping underwater can lead to drowning, and the shock alone can be dangerous for people with heart conditions. This first stage kills more people than hypothermia does.
  • Swim failure follows over the next few minutes. Cold rapidly weakens the muscles and reduces coordination, so even strong swimmers lose the ability to swim, keep their airway clear, or grab hold of a rescue line.
  • Hypothermia develops more slowly, over tens of minutes, as the body’s core temperature falls. It causes shivering, confusion and eventually unconsciousness.
  • Post-rescue collapse can occur during or after being taken from the water, as changes in circulation and body position strain the heart.

Understanding that the immediate danger is the initial shock, not the slower onset of hypothermia, changes how you should react.

Surviving Immersion in Cold Water

If you fall into cold water, your instinct will be to thrash and swim hard. Fighting that instinct in the first moments greatly improves your chances.

  • Do not panic and do not try to swim immediately. Let the cold water shock pass. Concentrate on floating, keeping your airway clear, and getting your breathing under control. This usually takes a minute or two.
  • Float to live. Lean back, keep your head above water, and spread your arms and legs to stay afloat while the gasping subsides. A lifejacket makes this far easier and is why wearing one is so vital.
  • Once your breathing settles, decide whether to swim for safety or stay put. If help is coming or you cannot reach safety, staying still conserves heat and energy. If you must swim, do so while you still have strength, before cold weakens your muscles.
  • Keep as much of your body out of the water as you can by climbing onto anything that floats, since heat is lost fastest by immersion.

The single biggest factor in surviving cold water is wearing a lifejacket, because it keeps your airway clear while the shock passes and you regain control.

Recovering Someone From Cold Water

Getting a cold casualty back on board is only part of the job; how you do it matters medically.

  • Keep them horizontal during and after recovery where possible. After time in cold water, lifting someone vertically can cause a sudden, dangerous drop in blood pressure.
  • Handle them gently. A very cold heart is sensitive, and rough handling can trigger dangerous rhythms.
  • Get them out of the wind and wet. Remove wet clothing, insulate them with dry layers or a survival bag, and shelter them from the cold.
  • Warm them gradually, not with sudden intense heat. Skin-to-skin contact and insulation help; avoid rubbing the limbs vigorously.
  • Monitor them closely, as the effects of cold can continue to develop after rescue. Seek medical help for anyone who has been significantly chilled.

Recognising and Treating Hypothermia

Hypothermia sets in as the body’s core temperature drops, and its signs progress predictably.

  • Early: intense shivering, cold and pale skin, clumsiness and slurred speech.
  • Worsening: shivering may stop, with increasing confusion, drowsiness and slowed breathing.
  • Severe: unconsciousness, and the casualty may appear lifeless while still being alive.

Treatment is to prevent further heat loss, warm the person gradually, keep them still and horizontal, and get medical help. Never assume a very cold, apparently lifeless casualty cannot be revived; treat them and seek professional help urgently.

Preparation is the best protection: wear a lifejacket, dress for the water temperature rather than the air, and stay clipped on in rough conditions.
